How to edit, save and print your forms:

Word Documents:

Editing a Word Document

These forms are protected Word documents. They allow a user to complete the form using Microsoft Word (or equivalent program).Download the form from the website, save it to your computer and then complete the form. You can enter data on each of the fields available for editing. A black box appears or a checkbox appears highlighted when you are on a field that accepts data. To fill in a checkbox, click in that box. To uncheck a checkbox, click the box again.

Saving and editing a PDF Form
To save a copy of a PDF form on your computer, click on the "Save a Copy" button on the Acrobat Reader tool bar (not your browser's tool bar). Caution: Acrobat Reader does not enable you to save information you have filled-in on your form or e-mail a completed form to another person. When you close the form, all data you have added will be cleared. You must print the form prior to closing it if you want filled-in information to print. If you would rather, you can simply print the blank form, and then enter your information by hand or typewriter.

Printing a PDF Form
To print a PDF form, click on the Print Button on the Acrobat Reader tool bar (not your browser's tool bar). Acrobat Reader's printing options are similar to those found in most software programs. Note: Some printers have issues with printing PDF format. If your document looks fine onscreen but doesn't print properly, try clicking on the "Print as image" checkbox on the Print dialog box. In many cases, this will resolve any issues.
